If you are unable to come to an Engagement Forum, or did not get to say everything you wanted to at the forum you attended, we’d like to invite you to complete the online survey. We are very keen to hear what you think the NDIS should look like.

Please remember you don’t have to answer all the questions – only answer the ones you want to make a comment about.
